The patient at the centre of the healthcare system – a need for coordinated care resulting from the increasing prevalence of lifestyle diseases. Cardiovascular diseases, diabetes and other morbidities requiring continuous treatment, prophylaxis, and monitoring therapy results. Key challenges: integrating the activity of primary care, outpatient, and inpatient hospital care, increasing preventive efforts and improving early diagnostics capabilities.
